Dato' Haji Azmi was born on 7 April 1948 at Kubang Rotan, Alor
Star, Kedah. He received his primary education at Sekolah Melayu
Kubang Rotan and then continued his secondary education at Mecca.
In December 1975, he received his Doctorate from University of Ain
Sham, Cairo.
Career
- Trainee Doctor at Kuala Lumpur Hospital (1976)
- Medical Officer at Alor Star Hospital (1977)
- Hospital Director and Health Officer of Baling District (1978-1982)
- Seconded to Pilgrims Fund Board as Director of Health (1985-1988)
- Director of Health Pilgrims Fund Board (December 1988-August

- Deputy Manager (Welfare and Health) at Pilgrims Fund Board (September
1995 - 30 September 1996)
- Senior Manager (Pilgrim) of Pilgrims Fund Board (1 October 1996

- Head of Malaysian Medical Team to the Holy Land for 12 Haj
seasons.
- Head of Pilgrims Welfare Team for 2 haj seasons (1995-1996)
- Head of Malaysian Pilgrims (1997 - present).

He was also chosen as 'Pentadbir Islam Terbaik' during Federal
Territory's Ma'al Hijrah celebrations in 1991. In recognition of
his achivements, Dato' Haji Azmi was bestowed with the B.C.K. from
DYMM Sultan Kedah (20 January 1982), K.M.N. from DYMM Seri Paduka
Baginda Yang DiPertuan Agung (8 June 1985), A.M.K. from DYMM Sultan
Kedah (19 January 1992), D.S.D.K. from DYMM Sultan Kedah (15 February
1998), J.S.M. from DYMM Seri Paduka Baginda Yang DiPertuan Agung
(2000) and the latest is the 'Anugerah Tertinggi Urusan Haji' from
the Saudi Arabian Government (February 2003).
